DECLARATION OF ELIGIBILITY AND PUBLICITY RELEASE

I, the undersigned ("Winner"), hereby declare and certify as follows in connection with the Kunes America 250 Giveaway (the "Promotion") sponsored by Kunes Country Automotive Management, Inc., 2141 E. Geneva Street, Delavan, Wisconsin 53115 ("Sponsor"):

1. DECLARATION OF ELIGIBILITY.

(a) I am a legal resident of the United States of America, and I am at least eighteen (18) years of age. My date of birth is:

(b) I am not an employee, officer, or director of Sponsor, its parents, affiliates, subsidiaries, representatives, consultants, contractors, legal counsel, advertising firms, public relations firms, vendors, OEMs, marketing agencies, or website providers (collectively, "Promotion Entities"), nor am I a member of the immediate family (spouse, parent, sibling, or child, regardless of where they live) or a person living in the same household as any such individual.

(c) I entered the Promotion in compliance with the Official Rules during the Promotion Period (July 1, 2026, through August 31, 2026, at 11:59:59 p.m. CT). I did not use any automated, bulk, or third-party entry method, and I did not submit any copies, facsimiles, or mechanical reproductions of the entry form.

(d) All information I provided in connection with my entry, including my name, address, telephone number, email address, and age, is accurate, complete, and truthful.

(e) I have reviewed the Official Rules for the Promotion and agree that I have complied with all requirements therein. I understand that my prize is contingent upon my fulfillment of all requirements set forth in the Official Rules and this Declaration.

2. PRIZE ACKNOWLEDGMENT.

I acknowledge that I have been selected as the winner of the following prize (check applicable prize):

• Prize 1 at Kunes Valley GMC — 2026 GMC Terrain AT4, stock #51T267, VIN 3GKALYEGXTL489311, Silver, 1.5L DOHC, 8-speed AT, AWD, MSRP $41,890 + $350 MN Dealer Doc fee + $60 E-File fee for a Verifiable Retail Value of $42,300.
• Prize 2 at Kunes Apple Valley Buick GMC — 2026 GMC Terrain Elevation, stock #52T391, VIN 3GKALUEG2TL480384, White, 1.5L DOHC, 8-speed AT, AWD, MSRP $35,590 + $350 MN Dealer Doc fee + $60 E-File fee for a Verifiable Retail Value of $36,000.

I understand that the prize Verifiable Retail Value listed above includes the applicable Dealer Service fee, Dealer Doc fee, and DMV E-File fee associated with that state and represents the amount that will be reported to me on IRS Form 1099-MISC. I understand that I am solely responsible for payment of all federal, state, and local income taxes arising from receipt of the prize, Wisconsin sales and use tax assessed on the Verifiable Retail Value of the vehicle at the time of title transfer, and all DMV title and license fees. Illinois winners may also be responsible for a 4.95% flat income tax on the Verifiable Retail Value of the vehicle. I acknowledge that no transportation to or from the dealership is included. I understand that the prize cannot be substituted, assigned, transferred, or redeemed for cash, and that no insurance is provided as part of the prize.

3. RELEASE OF LIABILITY.

In consideration of being awarded the prize and for other good and valuable consideration, the receipt and sufficiency of which I hereby acknowledge, I, on behalf of myself and my heirs, executors, administrators, legal representatives, successors, and assigns, hereby irrevocably release, discharge, and hold harmless Sponsor, Promotion Entities, and all of their respective affiliates, representatives, consultants, contractors, legal counsel, advertising and public relations firms, promotional, fulfillment, and marketing agencies, website providers, webmasters, and their respective officers, directors, shareholders, employees, representatives, and agents (collectively, the "Released Parties") from and against any and all claims, demands, actions, causes of action, losses, damages, liabilities, costs, and expenses (including attorneys' fees) of any nature whatsoever, whether known or unknown, arising out of or relating to: (a) my participation in the Promotion; (b) the award, acceptance, possession, use, or misuse of the prize; (c) any injury, death, loss, or damage to person or property arising in connection therewith; (d) any breach or alleged breach of any warranty, guarantee, or representation relating to the prize; (e) any claims based on rights of publicity, defamation, or invasion of privacy; or (f) any other matter relating to the Promotion or prize. I understand that the manufacturer warranty applicable to the prize vehicle, if in force, will be transferred to me pursuant to its terms.

4. PUBLICITY RELEASE.

Except where prohibited by law, I hereby grant to Sponsor and the Released Parties the perpetual, irrevocable, royalty-free, worldwide right and license to use, publish, broadcast, display, and reproduce my name, city and state of residence, photograph, portrait, picture, voice, likeness, biographical information, statements, and Promotion entry, in any and all media now known or hereafter developed — including but not limited to print, television, radio, the Internet, social media platforms (including Facebook, Instagram, X, and YouTube), and digital or electronic media — for advertising, trade, promotional, and publicity purposes in connection with the Promotion, without any additional consideration, compensation, notice, permission, or approval. I acknowledge that I will have no right to review, approve, or object to the manner in which my name or likeness is used in accordance with this grant. The winner can, in writing, opt-out of this publicity release at anytime without any effect to the awarded prize.

5. COMPLIANCE WITH OFFICIAL RULES; SPONSOR'S DECISIONS FINAL.

I acknowledge that I have read, understand, and agree to be bound by the Official Rules of the Promotion in their entirety. I agree that the decisions of Sponsor in all matters relating to the Promotion, including my eligibility and the award of the prize, are final and binding.

6. PRIZE DELIVERY SCHEDULING.

I understand that I am required to schedule delivery of the prize within seven (7) days of being notified that I am a winner, as set forth in the Official Rules. I agree to contact the applicable dealership at the number or email address provided by Sponsor to arrange a mutually agreeable delivery date and time. I understand that failure to schedule delivery within the required period may result in forfeiture of the prize.

7. GOVERNING LAW.

This Declaration shall be governed by and construed in accordance with the laws of the State of Wisconsin, without giving effect to any choice of law or conflict of law rules. Any dispute arising from this Declaration shall be subject to the dispute resolution provisions set forth in the Official Rules.

8. ACKNOWLEDGMENT.

I have read this declaration of eligibility and publicity release in its entirety. I understand that by signing below I am giving up certain legal rights, including the right to make certain claims against the released parties. I sign this declaration voluntarily and of my own free will.
